In the case of treatment of nasal bleeding before electrocautery, resection of the septum and resection of nasal polyps. Also used before tonsillectomy to inhibit the gag reflex and anesthesia of the injection site.

As an additional anesthesia before opening a peritonsillar abscess or in front of the antrum of the maxillary sinus.

Anesthesia before washing the sinuses.

Endoscopic and instrumental examinations

Pharyngeal anesthesia before the introduction of various tubes through the nose or mouth (gastroduodenal probe, Sengstaken probe).

Replacing the tracheotomy tube.

obstetrics and gynecology

Anesthesia of the perineum to perform episiotomy. Removal of stitches. Anesthesia of the surgical field during vaginal operations or operations on the cervix.

Dermatology

Anesthesia of the skin and mucous membranes with minor surgical interventions.

Contraindications.

Hypersensitivity to the components of the drug, as well as to other amide topical anesthetic drugs. If gypsum is used as impression material during the removal of prints of a tooth row, Lidocaine Health is contraindicated because of the risk of aspiration.

Dosage and administration.

The dose of the drug depends on the indications and the size of the surface to be anesthetized.

With each spray of 1 portion of the spray, 4.8 mg of lidocaine (1 dose) is emitted to the surface.

In order to avoid high concentrations of the drug in plasma, it is important to apply the lowest dose that provides a satisfactory effect.

1-2 sprays are usually sufficient, although 15-20 sprays or more are used in obstetrics (maximum dose is 40 sprays / 70 kg body weight).

Approximate doses for various indications. Indications Dose (number of clicks) Dentistry 1-3 Maxillofacial surgery 1-4 Otolaryngology 1-4 Endoscopy 2-3 Obstetrics 15-20 Gynecology 4-5 Dermatology 1-3

With the help of a cotton swab soaked in Lidocaine-Health, the preparation can be applied on large surfaces.

Children

In dentistry or maxillofacial surgery, Lidocaine Health can be applied to children by swabbing, thus avoiding the risk of inhaling the drug, as well as a burning sensation, which is usually a side effect of the drug. Children under 2 years old can use the drug in the same way.

For children as young as 12, the maximum dose is 3 mg / kg.

Adverse reactions.

Local effects

When using the drug, a burning sensation may occur, which disappears after the appearance of the anesthetic effect (within 1 minute).

At the site of application of the drug, transient erythema, edema and decreased sensitivity may occur.

Allergic reactions

Very rarely, allergic reactions can develop: urticaria, angioedema, bronchospasm, in extremely severe cases - shock.

In the case of hypersensitivity reactions, use of the drug should be discontinued.

System effects

If Lidocaine Health is used according to the instructions, the frequency of development of systemic effects is extremely low, since the amount of active substance that can reach the blood circulation is very small. When using high doses and in the case of rapid absorption of lidocaine or with hypersensitivity, idiosyncrasy, or decrease in tolerance, the following side effects may develop:

CNS reactions: agitation, depression, nervousness, dizziness, drowsiness, spasms, loss of consciousness, respiratory paralysis;

cardiovascular reactions: arterial hypotension, myocardial infarction, bradycardia, cardiac arrest.

Overdose.

In case of overdose, adverse reactions from the central nervous system and the cardiovascular system may occur, as described in the “Adverse Reactions” section.

If necessary, fresh air should be provided, oxygen should be supplied and / or artificial respiration should be carried out.

Seizures resulting from overdose should be eliminated with succinyl-choline (1 mg / kg body weight) and / or diazepam (1 mg / kg body weight IV). Since succi-nilcholine can cause respiratory arrest, the drug can be used only by specialists who have experience in conducting endotracheal intubation and in treating patients with respiratory arrest.

You can also use short-acting barbiturates, thiopental.

If symptoms occur on the part of the cardiovascular system (bradycardia, conduction disturbance), atropine (0.5-1 mg intravenously) and sympathomimetics should be prescribed.

With ventricular fibrillation and cardiac arrest, immediate resuscitation is indicated.

Dosage for children should be appropriate for the age and body weight of the patient.

Use during pregnancy or lactation.

No strictly controlled clinical studies of the safety of the drug during pregnancy have been conducted. However, lidocaine was used for a long time and did not harm the patient's health.

Preclinical studies did not reveal the toxic effect of lidocaine on the development of the fetus.

In the absence of a safer method of treatment, Lidocaine Health can also be used during pregnancy.

Lidocaine is excreted into breast milk, however, when using conventional therapeutic doses, the amount of the drug given is so paltry that it does not harm the baby who is breastfed.

Children.

The drug is used in pediatric practice (see section "Dosage and administration").

Lidocaine Health is not recommended for local anesthesia before tonsillectomy and adenotomy in children under 8 years of age.

Special security measures.

Do not allow lidocaine to enter the respiratory tract (risk of aspiration).

As far as possible when spraying an aerosol, the bottle should be in a vertical position. Avoid spraying the eyes.

When removing the impression of the dentition or when placing an X-ray film, the preparation can be used only when elastic impression materials are used.

Before using the drug Lidocaine-Health during operations on the pharynx and nasopharynx, it should be borne in mind that by suppressing the gag reflex and reaching the larynx and trachea, lidocaine inhibits the cough reflex, which can cause bronchopneumonia. This is especially important when using the drug for children, since they have more frequent swallowing reflex.

Features of application.

Caution should be applied to the drug on the back of the pharynx.

With transbuccal administration, there is a risk of dysphagia and subsequent aspiration, especially in children. Through the insensitivity of the tongue and cheek mucosa there is a risk of injury from a bite.

Lidocaine is rapidly absorbed through the mucous membranes (especially the tracheal mucosa), as well as through the injured skin. This should be taken into account, especially when the drug is applied to large areas or used for children.

Care should be taken when applying the drug to patients on the affected mucous membranes and / or infected areas.

Care should be taken when using the drug for patients with epilepsy, as well as for bradycardia, conduction disturbance, liver dysfunction, shock, especially if the absorption of a significant amount of the drug is expected on the basis of the applied dose and the treated area.

Attenuated patients, elderly patients, seriously ill patients and children should be given lower doses of the drug according to their age and general condition.

For children under 2 years old, it is advisable to apply Lidocaine Health with a cotton swab soaked in with the preparation.

The ability to influence the reaction rate when driving or working with other mechanisms.

There are no data on any effect of the drug on the ability to drive vehicles and work with other mechanisms.

Interaction with other drugs and other types of interactions.

Care must be taken when applying lidocaine to patients who receive type 1 antiarrhythmic drugs (for example, tocainide) because of the risk of cumulative toxic effects.

With simultaneous use with class IA antiarrhythmic drugs (including quinidine, procainamide, disopyramide), the QT interval is prolonged and in very rare cases the development of AV blockade or ventricular fibrillation is possible.

Phenytoin enhances the cardiodepressive effect of lidocaine.

With simultaneous use with procainamide delusions, hallucinations are possible.

Lidocaine can enhance the action of drugs that cause the blockade of neuromuscular transmission, because the latter reduce the conductivity of nerve impulses.

Against the background of glycoside intoxication, lidocaine can increase the severity of AV blockade.

Ethanol enhances the inhibitory effect of lidocaine on breathing.

Pharmaceutical interaction.

With simultaneous use increase the concentration of lidocaine in the serum of such drugs: aminazine, cimetidine, propranolol, peptidine, bupivacaine, quinidine, di-pyramid, amitriptyline, imipramine, nortriptyline.

Pharmacological properties.

Pharmacodynamics.

Lidocaine is a membrane stabilizing agent for the amide group for local anesthesia. It inhibits the sensory nerve endings of the skin and mucous membranes, that is, causes a reversible inhibition of the conductivity of the tissue elements of the nerve cells (neuron, ac-son, synapses).

The mechanism of action of local anesthetics is to inhibit ion flows through neural membranes, which are mandatory for the formation of a irritant.

Lidocaine inhibits stimulant-activated transient increase in permeability for sodium ions and to a lesser extent reduces passive permeability for potassium and sodium ions, thereby stabilizing neural membranes. Lidocaine reduces the degree of depolarization, which occurs in response to a physiological stimulus, as well as the amplitude of the action potential, and inhibits nerve conduction.

Among the various sensory modes of action, local anesthetics primarily suppress pain sensitivity, accompanied by inhibition of the sensation of heat and tactile sensations. Absorbed after topical administration, lidocaine may cause agitation or depression from the central nervous system. Its effect on the cardiovascular system can manifest itself in the form of conduction disturbances and peripheral vasodilation.

Pharmacokinetics.

After topical application, lidocaine penetrates the tissue and has a local anesthetic effect.

The effect of lidocaine develops within 1 minute and lasts from 5 to 6 minutes. The subjective symptoms of insensitivity disappear slowly, within 15 minutes.

Lidocaine is rapidly absorbed when applied to the mucous membrane and damaged skin, but is poorly absorbed when applied to healthy skin. The rate of absorption and the amount of active substance that enters the bloodstream depends on the dose, type, size and condition of the surface on which the drug is applied (skin or mucous membrane), as well as the duration of exposure.

Transdermal administration of 500 mg lidocaine provides therapeutic blood levels of the drug. The maximum concentration of lidocaine in the blood plasma is achieved within 1 h after the use of the drug. When applying this dose, the concentration of lidocaine in the serum remains in the therapeutic range for 7-8 hours. The amount of active substance, which is used in any case, should not exceed 300-350 mg.

Lidocaine is metabolized in the liver. First, it is dealkylated, and then hydrolyzed. Both unchanged drug and its metabolites are excreted mainly by the kidneys. The half-life of lidocaine is 1.6 hours.
